The below new patient paper work form must be completed online 48 hours prior to your scheduled appointment. If not, then sorry we will cancel your appointment.
We need this amount of time to pull your records and for Dr. Millman to review your submitted info before your appointment so that he can be better prepared to help you. It takes a significant amount of front office effort to compile your digital chart with all of your pertinent and relevant records. Also, remember there is a no show fee and “not timely” cancellation fee as well for this reason. See our policy below for details
